I'm starting today – what should I do to prepare?

We see a lot of posts from people who are about to start taking weight loss medication, and are wondering what "you wish you knew when you started." First off, take a deep breath. You got this. And here are a few tips to help you out as you begin your journey.

Worried about the needle? It's normal to be freaked out by the idea of injecting yourself, but I swear it's easy, painless, and you'll quickly get used to it. Follow the instructions from your provider, and watch a video about how to self-administer a subcutaneous injection. Here's a good one from the VA that also has some good pointers (no pun intended) like letting the vial to come up to room temperature before injecting:

Drink lots of water. These medications can be dehydrating, water is your friend...it will help the weight come off and help stave off fatigue. Google for guidelines, but I have a big 40oz hydroflask and I try to have 2-3 of those a day. It's also recommended to replenish your electrolytes; I personally have one of these LMNT packets a day in my water bottle.

Make sure you are getting enough protein. Again, like water, search online for guidelines for your own height and weight/target weight. This is more of an ongoing thing than a "just starting" thing, but not getting enough protein (and calories in general) can get in the way of progress and mean you lose more lean body mass than you’d like. I supplement with 12oz of almond milk mixed with a scoop of this protein powder most days.
Avoid big, heavy meals the day before and the day of injecting. You'll get a feel for what your body can handle once you get into the swing of things, but to start, best to lean on lighter meals.
You might not feel anything on the starter 2.5mg dose, and that’s okay. It’s really just meant to get your body acclimated to the drug. You’ll read lots of accounts of people feeling the food noise disappear immediately and having very dramatic effects, but this is not as common as the subreddits would make it seem.
Be prepared for potential mild side effects (though many people have none!):
- Nausea: Have some ginger tea or ginger chews/candy -- I really like these organic ginger Tummydrops. I also like this chewable Emetrol, it tastes good and works for when you might need a little more than a tummydrop. Personally, I've never felt more than a tiny bit queasy, nothing too bad.
- Tiredness: Plan to take it easy on your first injection day. This is the biggest side effect I face, and it hits me pretty much every week the day or two after injection. As mentioned above, drinking lots of water and replenishing electrolytes can help. If anyone has any other suggestions, let me know and I can share them!
- Constipation: Things can, um, slow down a bit down there. Make sure you have Miralax or another gentle laxative available. People swear by MagO7 -- a magnesium supplement that has a laxative effect, but I haven't tried it, as I haven't experienced this, thankfully.

Help! I can't understand my prescription label. What's my concentration?

Medication labels can look confusing with all their numbers and text, but there's one key piece of information you need: the concentration.

Think of concentration as the strength of your medication. Just like a strong cup of coffee has more caffeine per ounce than weak coffee, a higher concentration medication has more medicine packed into each milliliter of liquid.

What you're looking for: A number followed by either mg/mL or mg/0.5mL (like 10mg/mL or 5mg/0.5mL). Our calculators support both via a unit toggle above the concentration field, so you can enter what you see on the label and we'll convert as needed.

Simple example: If your label says "15mg/mL", that means:

Every 1mL (which equals 100 units on your syringe) contains 15mg of medicine
So for a 15mg dose, you'd inject exactly 1mL (100 units)
For a 7.5mg dose, you'd inject 0.5mL (50 units)

The examples below show you exactly where to find this concentration number on different types of labels.

Standard Format

Most common format showing concentration as X mg/mL:

In this format, the concentration is clearly stated as mg per mL. This is the easiest format to read as no calculation is needed.

The concentration is 20 mg/mL, so you would enter "20" in the calculator.

Partial mL Format (mg/0.5mL)

Sometimes pharmacies list concentration per 0.5mL instead of per full mL. You can now use the unit toggle to select mg/0.5mL and enter the number exactly as shown on your label.

Since 0.5mL = half of 1mL, double the mg value

Best practice: Use the mg/0.5mL toggle and enter "2" to avoid mistakes — the calculator will show both mg/mL and mg/0.5mL in results.

Format with Additives

Many pharmacies add extra ingredients like B12, niacinamide, or glycine:

Key rule: Only the tirzepatide or semaglutide concentration matters for dosing. Ignore all the other ingredients and their numbers.

How to find the right number:

Look for "Tirzepatide" or "Semaglutide" in the ingredient list
Find which position it's in (first, second, etc.)
Use the concentration number in that same position
In this example: Tirzepatide is listed first, so use the first number (8 mg/mL)

The tirzepatide concentration is 8 mg/mL, so you would enter "8" in the calculator.
(Ignore the niacinamide and its concentration—it doesn't affect your dosing)

These vials look small! Did my pharmacy send me the right amount?

We hear this one a lot on Reddit. The vials are tiny, and often less than half full (if that!), and you might be thinking – "Hold on a sec, how is this really going to last me 4 weeks, or more?!"

It's common for vials of injectable medication to appear smaller than you might expect, especially because these medications are often highly concentrated. The amount in the vial may seem small, but it's typically enough for the prescribed number of doses based on your current treatment plan.

Remember: 5ml is just about a teaspoon, and most of these vials are 2 to 4mL, so it's not a lot of liquid at all. It's potent stuff!

Question 4

How long will my medication last?

Accepted Answer

There are two scenarios here -- a sealed, never used vial, and a vial once it's first used. Let's look at each separately.

Sealed Vial

For an unopened, never-punctured vial: with proper refrigeration, the medication will last at least until the "Beyond-Use Date" (BUD) printed on the vial. This date can range from one month to one year from the compounding date.

The BUD is the date beyond which the safety and effectiveness of a compounded drug can no longer be guaranteed. However, these dates aren't based on testing of your specific medication, but rather on USP United States Pharmacopeia guidelines, state regulations, and published studies. This means a pharmacy might set a BUD one year out while another sets it just one month out - this difference often reflects state regulations rather than the actual medication inside.

Many people report successfully using their properly refrigerated compounded GLP-1s beyond the listed BUD without issues, but this is done at your own risk. The main risks are:

Loss of effectiveness - the medication may not work as well
Contamination - which could be dangerous

Warning signs to watch for: If your medication appears discolored, cloudy, or contains any particles, that's a sign that something has gone bad. Discard the vial (or check with your provider first).

Opened/punctured vial

Once the vial has been opened and used for the first time, with the rubber cap punctured by your syringe, a different "timer" begins.

The standard recommendation is that once punctured, a vial should be used within 28 days. This is a conservative safety guideline designed to minimize the risk of contamination.

However, many people successfully use their vials for up to two months (8 weeks) without issues, provided they follow proper handling protocols:

Always wipe the top of the vial with an alcohol swab before and after each puncture - always!
Keep the vial refrigerated between uses - in a closed container to avoid contamination
Never reuse needles - use a fresh, sterile syringe for each injection
Avoid touching the rubber stopper with your fingers

The primary risk of using a vial beyond the 28-day guideline is contamination - bacteria or other microorganisms could potentially enter the vial through the puncture site, even with proper handling. While many people report no problems extending use to two months, going much beyond that increases the contamination risk significantly.

Ultimately, this is a personal risk tolerance decision. If you choose to extend beyond 28 days, pay close attention to any changes in the medication's appearance (cloudiness, discoloration, particles) or unusual side effects, and discard the vial (or check with your provider) if you notice anything that looks off.

Question 5

What can I do to minimize GLP-1 side effects?

Side effects on GLP-1 medications — whether you're on tirzepatide (Mounjaro, Zepbound), semaglutide (Ozempic, Wegovy), or Foundayo (orforglipron) — are common, especially early on during titration. But "common" doesn't mean you just have to white-knuckle it through them. Most side effects improve significantly as your body adjusts, and the biggest thing you need to know is that most of these have simple, practical fixes. Don't suffer in silence.

General Tips (These Apply to Almost Everything)

Before we get into the specific side effects, a few things that help across the board:

Titrate slowly. There is absolutely no shame in staying at a lower dose longer than the "standard" schedule. If your provider gives you flexibility, use it. Rushing to higher doses is one of the most common reasons people have rough experiences. (Not sure when to move up? Eat smaller, more frequent meals. Big meals are harder on your system when you're on these meds. Smaller portions, more often, is your friend.
Water is your best friend. Seriously. You need more of it than you think. I use a 40oz Hydro Flask and try to get through 2-3 of those a day. It helps with almost every side effect on this list.

Nausea

Nausea is the most common side effect, especially during early titration and after each dose increase. The good news is it usually settles down within a few days to a week at each dose level — your body is adjusting.

A few things that help:

Avoid rich, fatty, greasy, or spicy foods for the first few days after a dose increase. Bland is your friend. Toast, crackers, simple stuff. You can go back to eating normally once your stomach settles.
Ginger is genuinely useful. Ginger tea, ginger chews, ginger candy — it's not just an old wives' tale, it actually helps with nausea. I keep Tummydrops on hand at all times.
Eat something small before or shortly after your injection. An empty stomach often makes nausea worse.
Try taking your injection before bed. A lot of people find they sleep through the worst of it and wake up feeling fine. Worth experimenting with if nausea is a consistent problem.

Constipation

These medications slow down gastric emptying — that's actually part of how they work. But the downstream effect is that things can get pretty sluggish. This is extremely common and worth getting ahead of proactively rather than waiting until it's a problem.

Fiber matters. Make sure you're eating enough fruits, vegetables, and whole grains. A lot of people eating less overall also end up eating less fiber, which compounds the issue.
Magnesium supplements are popular in the GLP-1 community. MagO7 is one you'll see mentioned a lot — it's magnesium oxide with a gentle laxative effect. It works, but a couple of caveats: don't use it long-term without checking with your doctor, and it's not safe for people with kidney disease. Magnesium citrate or glycinate may be better tolerated for some people. Ask your provider what makes sense for you.
Miralax is a reliable fallback. It's gentle, over-the-counter, and it works. Keep some in the cabinet.
And again — water. Dehydration makes constipation significantly worse.

Fatigue and Tiredness

This one catches people off guard. Fatigue can hit pretty hard, especially in the day or two after a dose increase. A few things to keep in mind:

It's often a nutrition issue. If you're eating much less than usual, your body may simply not have enough fuel. Make sure you're getting enough calories overall.
Protein matters a lot. Not getting enough protein is one of the most common culprits behind persistent fatigue on these meds. If you're struggling to hit your protein goals through food alone, supplementing with a protein shake is a practical option.
Electrolytes help here too. Fatigue is one of the first signs of electrolyte depletion. Consider an electrolyte drink or supplement, especially if you're also dealing with GI side effects.
Experiment with injection timing. Some people find that injecting on a Friday means they're draggy on the weekend, while others prefer that over being tired at work. Try different days and times to find what fits your life.
If fatigue is severe and persistent, talk to your provider. It could be a sign that you're titrating faster than your body is comfortable with. Slowing down is always an option.

Diarrhea

Less common than nausea or constipation for the injectables, but it happens — particularly around dose changes. If you're on Foundayo (orforglipron), diarrhea and indigestion are actually more prominent than nausea, so this section may be especially relevant for you. Either way, it's usually transient and resolves on its own.

Stick to bland, easy-to-digest foods when things are rough. You may have heard of the BRAT diet (bananas, rice, applesauce, toast) — those are fine starting points, but don't limit yourself to just those four. Brothy soups, plain crackers, boiled potatoes, plain chicken, and scrambled eggs are all gentle on the stomach and give you more nutrition. Get back to a balanced diet as soon as you can.
Stay hydrated — when you have diarrhea, you're losing fluids and electrolytes fast. This is when an electrolyte drink really matters.
If it's persistent rather than occasional, check in with your provider.

Headaches

Headaches on GLP-1 meds are usually a hydration issue. Your first move when you feel one coming on should be drinking a big glass of water.

They can also be related to blood sugar changes, especially early on in your treatment. This typically resolves on its own within the first week or two as your body adapts.

Injection Site Reactions

Some redness, mild itching, or a small bump at the injection site is normal and not a cause for concern. A few things that help:

Rotate your injection sites. Don't keep using the same spot. Your abdomen, thighs, and upper arms are all fair game — cycle through them. If you're new to self-injection, our getting started guide has a helpful tutorial video.
Let the medication come to room temperature before injecting. Cold medication stings more going in and is more likely to cause local irritation. Give your vial 15–30 minutes on the counter. (For more on proper storage and temperature, see our medication storage guide.)
If you develop persistent lumps, significant swelling, or anything that doesn't resolve in a day or two, mention it to your provider.

Acid Reflux and Sulfur Burps

This one doesn't get talked about enough. A lot of people on GLP-1s experience acid reflux, heartburn, or the notorious "sulfur burps" — burps that taste and smell like rotten eggs. It's unpleasant but it's common and usually manageable.

Avoid eating close to bedtime. Give yourself at least 2-3 hours between your last meal and lying down.
Elevate the head of your bed if reflux is waking you up at night.
OTC antacids (Tums, Pepcid, Prilosec) can help. If you're using them regularly, mention it to your provider.
The sulfur burps specifically tend to improve as your body adjusts to each dose level. They're gross but temporary.

Hair Loss

Some people notice increased hair shedding a few months into treatment. This is called telogen effluvium — it's triggered by rapid weight loss (not the medication itself), and it's usually temporary. Your hair growth cycle catches up eventually.

Getting enough protein is the most important thing you can do here. Inadequate protein during weight loss makes hair shedding worse.
Slowing down your rate of weight loss can help — another reason not to rush titration. Some people also find that split dosing helps manage side effects while maintaining efficacy.
Collagen supplements (particularly marine collagen) have some emerging evidence for supporting hair during telogen effluvium, though the research is still early. Some people swear by them — worth trying, but don't expect miracles.
If it's significant or persistent, talk to your provider. Checking iron, thyroid, and vitamin D levels is a reasonable step.

When to Call Your Doctor

Most side effects are manageable at home with the tips above. But some things are "call now" situations, not "mention it at my next appointment":

Severe or persistent vomiting that prevents you from keeping food or water down
Signs of dehydration (extreme thirst, dark urine, dizziness)
Severe abdominal pain — especially if it's intense, persistent, and radiating to your back, accompanied by nausea, vomiting, or fever. This can be a sign of pancreatitis. Don't wait on this one — go to the ER.
Any side effect that feels genuinely unmanageable and is affecting your quality of life

One more thing: if nausea or vomiting is consistently rough despite the tips above, ask your provider about ondansetron (Zofran) — it's an anti-nausea prescription that many GLP-1 prescribers will write alongside the medication, especially during titration. There's no reason to suffer when there's a straightforward prescription option.

Don't tough it out if something feels really wrong. Your provider can adjust your dose, change your titration speed, or help troubleshoot. That's what they're there for.

The Big Picture

The first few weeks and the days after dose increases are when side effects are typically at their worst. For most people, things settle down a lot once you've been at a given dose for a while. How do I store my medication correctly?

Injectable medications like tirzepatide or semaglutide need to be stored in the refrigerator. Make sure the medication is kept at a temperature between 36°F and 46°F (2°C to 8°C) (that's standard refrigerator temperature.) Be careful -- the back of the fridge can sometimes get down to freezing temps, which you want to avoid.

Key storage points:

Keep it refrigerated, but look out for the very back of the fridge which might get too cold
Do not freeze the medication (note: there's some debate about this point, but everyone agrees that the freeze/thaw cycle is bad, so if you decide to freeze, make sure it stays frozen until you are ready to use.)
Avoid keeping in direct sunlight or extreme heat
Can be left at room temperature (under 86°F) for up to a week or so if needed
Keep the vial in a closed container to protect from light and help avoid contamination

How Do I Travel with My GLP-1 Medication?

Traveling with injectable medication sounds a lot more complicated than it actually is. Millions of people fly with insulin, biologics, and GLP-1s every single day. TSA knows what these are. Hotels deal with requests like this all the time. You're not going to be the first person at customs with a vial and some syringes. Here's everything you need to know so you can just pack your stuff and go.

Keeping Your Medication Cold

Your medication needs to be refrigerated at 36–46°F (2–8°C) when possible, but you have more flexibility than you might think.

Room temperature tolerance varies by medication:

Tirzepatide — up to 21 days at up to 86°F
Semaglutide (Ozempic) — up to 56 days at up to 86°F
Wegovy — up to 28 days at up to 77°F
Compounded vials — varies by compounder, so check your pharmacy label. In practice, most compounded peptides are fine at room temp for a few weeks, similar to the name-brand stuff

One important rule: once a vial leaves the fridge, the clock starts. You can't put it back and reset the timer.

Avoid freezing your medication. The official guidance from manufacturers is to discard medication that's been frozen — the concern is protein denaturation, where the peptide molecules break down in ways you can't see. That said, plenty of people on Reddit report freezing their vials and noticing no loss of efficacy. The science says it should degrade, but real-world reports are mixed. Our recommendation: don't freeze if you can avoid it, and definitely avoid repeated freeze-thaw cycles. But if your vial accidentally froze once in transit, it's not necessarily ruined — just something to be aware of.

For travel, your two main options are evaporative cooling wallets and insulated bags with gel packs:

FRIO wallets use evaporative cooling — you wet them, they cool down, no ice needed. They're lightweight and great for short trips or warm-weather travel. The catch: they keep your medication cool relative to ambient temperature, not true refrigerator cold. Fine for stays within the room-temp tolerance window.
Insulated medical bags with gel packs can maintain actual fridge temps for 8–24+ hours depending on quality. Better for longer travel days or if you want true cold. This is what I personally use for travel and it's worked well for me.

A few tips regardless of which you use: put a cloth barrier between any ice pack and your vial to prevent freezing, never leave medication in a hot car, and never put it in checked luggage (cargo holds can freeze, and if the bag gets lost, so does your medication).

Hotel Tips

Most hotels will provide a mini-fridge for medical reasons at no charge, even if the room doesn't normally include one. Call ahead and mention you need it for temperature-sensitive medication. Most front desks are used to this and will accommodate you without any fuss.

If your room already has a mini-fridge, use the middle shelf. The back wall can get cold enough to freeze things, especially in older or poorly calibrated units. Wrapping your vial in a washcloth gives a little insulation from cold spots.

If you're in a pinch, hotel ice machines work as a backup — bag up some ice, wrap your medication in a cloth barrier so it doesn't touch the ice directly, and you've got a decent makeshift cooler for the night.

Flying with Your Medication (TSA)

Good news: TSA is not your enemy here. A few things to know:

Injectable medications are explicitly allowed through security checkpoints
They are exempt from the 3.4oz/100ml liquid rule — your vials do not need to go in the quart-size bag
Syringes and pen needles are allowed when accompanied by injectable medication
Ice packs and gel packs are allowed for medication cooling, even if they're frozen solid

TSA does not require a prescription label or a doctor's letter — but they strongly recommend medications be clearly labeled. For compounded vials especially (which don't have standard pharmacy branding), a doctor's letter is good insurance.

The smoothest way to get through security: put your medication and supplies in a separate clear bag, and when you get to the front of the line, just tell the officer you have medically necessary liquids before you start loading the bins. Takes two seconds and heads off any confusion.

If you have concerns or want guidance specific to your situation, call the TSA Cares helpline at 855-787-2227 at least 72 hours before your trip. They can walk you through what to expect and even arrange checkpoint assistance if needed.

International Travel

GLP-1 medications are not controlled substances in any major country. They're prescription medications — nobody's abusing tirzepatide recreationally. What does attract scrutiny is the injectable form: needles and syringes raise flags at customs in ways that a pill bottle wouldn't.

Get a doctor's letter. This is the single best thing you can do for international travel. A good letter includes:

Your name (matching your passport)
Medication name — brand and generic
Your dosage and that you must inject it
That it must be kept cold
Your doctor's credentials and contact info

For compounded medications, this is even more important since you won't have standard manufacturer packaging backing up your story.

Beyond the letter, keep everything in its original packaging with pharmacy labels when you can. And always declare your medications on customs forms when asked. The risk of not declaring far outweighs the minor inconvenience of declaring.

Most countries — the EU, UK, Canada, Mexico, Australia — are pretty straightforward as long as you have a doctor's letter and original packaging. You'll breeze through.

Japan is the exception. Japan has a strict system called Yakkan Shoumei (薬監証明) — a medicine import certificate. You can bring up to a one-month supply of injectable medications with a doctor's letter, but if you're bringing more than that, you need to apply to Japan's Ministry of Health, Labour and Welfare at least two weeks in advance. Don't find this out at Narita. Research it before you go.

One more thing: watch out for transit countries. If you have a layover, the rules of that country technically apply too. It almost never comes up, but worth being aware of.

And in general, don't bring six months of supply on an international trip. It can raise suspicion of intent to distribute, which is a headache you don't need.

Disposing of Needles While Traveling

This one matters more than people realize. Never throw used needles loose in a hotel trash can. Needlestick injuries to housekeeping staff are a real risk, and it's illegal in many jurisdictions.

The easiest solution is a small single-use travel sharps container. They're compact enough to toss in your toiletry bag, and you can grab a multi-pack for a few bucks on Amazon. You can bring a sealed sharps container back through TSA — they're allowed in carry-ons.

A popular alternative is a needle clipper — a tiny device that clips and stores the needle tip, making the remaining syringe body safe to dispose of normally. A lot of travelers love these because they're way smaller than a full sharps container. (The original BD Safe-Clip appears to have been discontinued, but there are good alternatives.)

Short answer: Yes - there are now two oral GLP-1s approved for weight loss (as of April 2026).

Oral Wegovy (semaglutide) is FDA-approved for weight loss. It was approved on December 22, 2025, and became available in early January 2026. It's the first oral GLP-1 actually approved for weight loss, not just diabetes.

The results are solid: in the OASIS 4 trial, people who stuck with it lost about 16.6% of their body weight at 64 weeks (about 13.6% on average when you count everyone who started). That's close to what you see with injectable Wegovy. The dose is 25mg once daily, and the introductory price is $149/month.

The downside: you still have to take it on an empty stomach and wait 30 minutes before eating or drinking anything. Same routine as Rybelsus. If you were hoping for a pill you could just pop with breakfast, this isn't quite it.

What about Rybelsus? It's still around, but it's only approved for diabetes, not weight loss. And now that oral Wegovy exists, there's really no reason to mess with Rybelsus for weight loss anymore.

What about oral tirzepatide (Mounjaro/Zepbound)? Eli Lilly is not developing an oral version of tirzepatide. Any company selling "oral" or "sublingual" tirzepatide is selling a compounded product that's not FDA-approved. Personally, I would avoid it.

Foundayo (orforglipron) — FDA approved April 1, 2026: Eli Lilly's Foundayo was approved on April 1, 2026, after receiving priority review. The ATTAIN-1 trial showed about 12.4% weight loss among completers (11.1% on an intent-to-treat basis) at the highest dose — roughly 25–27 lbs on average.

The appeal of Foundayo: you can take it any time, no empty stomach requirement, no waiting period. Take it with your morning coffee or with dinner. That's a real convenience advantage over oral Wegovy or Rybelsus.

On paper, the weight loss numbers are a bit lower than the injectable GLP-1s — but real-world compliance with a no-restrictions daily pill is likely to close that gap. And the ATTAIN-MAINTAIN trial showed it can help maintain weight loss after switching from injectables — people who switched from Wegovy maintained about 95% of their prior weight loss, and people who switched from Zepbound maintained about 80%. So one option is using a shot to lose the weight, then switching to Foundayo to maintain it without the needles.

On pricing: Foundayo starts at $149/month for self-pay at lower doses, up to $299/month at the highest dose through Lilly's Self-Pay Journey program ($349/month without it). If you have commercial insurance, Lilly's savings card brings it down to $25/month. Medicare Part D coverage kicks in at $50/month starting July 1, 2026. Prescriptions through LillyDirect started shipping April 6, 2026, with broad retail and telehealth availability expected shortly after.

What about Pfizer's danuglipron? They discontinued it in April 2025 due to liver toxicity concerns.

Bottom line: For most people, Foundayo is the better oral GLP-1 to start with. Yes, oral Wegovy produces slightly better weight loss in clinical trials — but trials measure what happens under ideal conditions. In the real world, a pill you can take with your morning coffee is a pill you'll actually take every day. The empty stomach + 30-minute wait that oral Wegovy requires is the kind of friction that kills compliance over months and years. Foundayo's convenience edge matters more than a few percentage points of trial efficacy if it means you stick with it. And at $25/month with insurance, the cost barrier is basically gone.

That said, oral Wegovy is still a solid option if you're already comfortable with the fasting routine (especially if you were on Rybelsus). And if you want the absolute best results regardless of how you take it? The injections are still king. But the pill options are finally here and actually worth considering.

When should I move up to the next dose?

The short answer: When your current dose stops working effectively for appetite suppression and weight loss, it's usually time to consider moving up.

But let's break this down a bit more.

What the name-brand schedules look like

The FDA-approved titration schedules are pretty standard across the board:

For semaglutide (Ozempic/Wegovy): Start at 0.25 mg per week and gradually increase to 2.4 mg per week for weight loss, titrating every four weeks. So you'd typically go 0.25mg → 0.5mg → 1mg → 1.7mg → 2.4mg, spending about a month at each dose.

For tirzepatide (Mounjaro/Zepbound): Start at 2.5 mg, then increase to 5 mg once a week for 4 weeks, followed by increases to 7.5 mg once a week. The full schedule is usually 2.5mg → 5mg → 7.5mg → 10mg → 12.5mg → 15mg, again with about 4 weeks between increases.

These schedules are designed to minimize side effects while your body adjusts to the medication, while getting up to a maximum dose relatively quickly.

Here's where compounding gets interesting

One of the biggest advantages of working with a compounding pharmacy is that you're not locked into those rigid manufacturer schedules. You and your doctor can create a custom titration plan that works for YOUR body and YOUR goals.

Maybe you need to go slower because you're sensitive to side effects. Maybe you want to micro-dose increases by going up in smaller increments. Or maybe you're responding well and want to move a bit faster. With compounding, you have that flexibility.

The "stay low as long as possible" approach

A lot of people try to ride out lower doses for as long as they can, and that's totally understandable. For tirzepatide especially, we see folks wanting to stay at 2.5mg or 5mg forever because they're seeing results and don't want to deal with increased side effects or higher costs.

Here's the reality though: this isn't going to work for everybody. Many people won't see the full weight loss potential of these medications until they reach higher doses. If you've been stuck at the same weight for several weeks, your appetite is creeping back, or you're finding it harder to stick to your eating plan, it might be time to consider moving up in dose. There's no shame in that!

Signs it might be time to increase

Weight loss has stalled for 4+ weeks (a week or two is not a stall)
Your appetite is returning to pre-medication levels
You're having more food noise and cravings
You're not feeling satisfied after meals like you used to
You're having to white-knuckle it through your eating plan again

The bottom line

Work with your doctor to find the right balance for you. There's no shame in needing higher doses — these medications are dose-dependent, meaning more medication often equals better results. The goal is finding your own personal sweet spot where you're getting maximum benefit with manageable side effects.

And remember, with compounding, you have way more flexibility to fine-tune your dosing than you would with the name-brand pens. Take advantage of that flexibility to create a plan that actually works for you.

While the FDA-approved protocols call for once-weekly injections, many people find that splitting their dose works better for their body and lifestyle. And with compounded medications, you have the flexibility to experiment with different dosing schedules under your doctor's guidance.

Why would you want to split your dose?

The main reasons people consider split dosing usually come down to managing side effects and maintaining more consistent levels of the medication in their system.

Side effect management: Many people find that taking a smaller dose more frequently reduces those unpleasant GI side effects like nausea, vomiting, and stomach upset. This stepwise approach may help reduce gastrointestinal symptoms such as nausea or bloating, which may be common with GLP-1 receptor agonists. So if you're normally on 5mg of tirzepatide weekly, you could simply do 2.5mg twice per week.

Things to keep in mind

It's not officially studied: While many healthcare providers are comfortable with split dosing, it's important to know that there is no medical or scientific evidence supporting a twice-weekly injection schedule of Ozempic, and doing so could lead to unpredictable results. The clinical trials that got these medications approved were all done with once-weekly dosing.

Work with your provider: This isn't something to try on your own. Your doctor needs to be involved in planning any changes to your dosing schedule, especially since everyone's body responds differently.

More injections means more work: Obviously, split dosing means more frequent injections, which some people find annoying or inconvenient. You'll also go through your medication supply faster in terms of actual injection events, even though you're using the same total amount.

Compounding gives you options: With name-brand pens, split dosing can be tricky because you're stuck with pre-filled amounts. With compounded medications, you have much more flexibility to create custom split dosing schedules that work for your specific needs.

Retatrutide Dosing Guide: What the Trial Data Tell Us

Retatrutide is Eli Lilly's investigational triple agonist (GLP-1 / GIP / glucagon). As of May 2026 it is not FDA-approved — it's still in Phase 3 trials. This page summarizes what the published trial data show about how retatrutide has been dosed in clinical studies, and what an approved label might plausibly look like if approval comes through.

What Makes Retatrutide Different

Retatrutide activates GLP-1 receptors (like semaglutide), GIP receptors (like tirzepatide), and glucagon receptors. The glucagon component is the new piece, and it's the most likely explanation for the differences from tirzepatide seen in the trial data — both the higher reported weight loss and a few side effects that haven't been seen at this rate with tirzepatide.

What the published data show:

Weight loss in TRIUMPH-4 (68 weeks): an average of 28.7% body weight reduction at the 12 mg target dose and 26.4% at 9 mg (efficacy estimand). For context, tirzepatide reported roughly 22.5% in similar populations.
Higher gastrointestinal adverse event rates than tirzepatide. At 12 mg in TRIUMPH-4: nausea ~43%, vomiting ~21%, diarrhea ~33%.
Heart rate increase reported in Phase 2 trials. A dose-dependent rise of about 6 bpm at 12 mg, peaking around week 24 and declining afterward — consistent with glucagon receptor activation.
Dysesthesia (skin tingling/sensitivity). Reported in ~21% of TRIUMPH-4 participants at 12 mg and ~9% at 9 mg. This effect appears more or less unique to retatrutide in this drug class.
Higher discontinuation due to adverse events at the top dose: 18% on 12 mg, 12% on 9 mg, 4% on placebo in TRIUMPH-4. Tirzepatide's pivotal trials reported closer to 6–7%.

The 9 mg dose is notable. In TRIUMPH-4 it produced nearly the same weight loss as 12 mg with materially less GI burden and a third lower discontinuation rate. If retatrutide is eventually approved, it's plausible that 9 mg emerges as a common maintenance dose rather than 12 mg.

How Retatrutide Was Dosed in the TRIUMPH Phase 3 Trials

Across the TRIUMPH Phase 3 program, participants started at 2 mg once weekly, with the dose escalated in 4-week intervals to a randomized target dose, then maintained for the remainder of the 68- or 80-week treatment period. TRIUMPH-4 specifically tested the 9 mg and 12 mg target doses (no 4 mg arm); other Phase 3 trials in the program also include 4 mg.

The published titration ladder for the program looks like this:

The starting dose in Phase 3 was standardized at 2 mg. In Phase 2, a 4 mg starting dose was associated with higher GI adverse event rates, which appears to be why Phase 3 used the lower start.
Each dose level was held for at least 4 weeks before escalation.
12 mg is the highest dose evaluated in Phase 3. No higher dose has clinical trial data.
Trial protocols permitted permanent dose reductions for participants experiencing gastrointestinal adverse events or perceived excessive weight loss (BMI ≤ 22 kg/m²).

If retatrutide is approved, the FDA-approved label could look similar to this ladder — but it could also differ in starting dose, escalation timing, available maintenance doses, or maximum dose. The trial protocol is not a label.

TRIUMPH-4 Reported Outcomes: 9 mg vs. 12 mg

Both arms started at 2 mg, both used 4-week titration intervals, both ran 68 weeks. Baseline averages: ~248.5 lbs and BMI 40.4. All figures below are from Lilly Medical's preliminary results disclosure for TRIUMPH-4.

The 12 mg arm produced more weight loss. The 9 mg arm produced essentially the same WOMAC pain reduction with about half the dysesthesia and a notably lower AE discontinuation rate.

A Hypothetical "Slow Titration" Schedule

A handful of compounding-pharmacy clinicians and online retatrutide communities have described a slower variant of the trial titration — extending each step from 4 weeks to 6 or 8 weeks, on the theory that retatrutide's higher GI burden and heart rate signal warrant a gentler ramp. No clinical trial has tested this approach.

Some users of compounded retatrutide describe starting at sub-trial doses — below 2 mg per week — and gradually working up. These schedules have no clinical trial support. No published trial has formally evaluated retatrutide doses below 2 mg as a sustained regimen.

A typical schedule reported in online communities looks something like this:

Because retatrutide is not approved, any retatrutide currently available in the US comes from compounding pharmacies or research-peptide channels. The FDA does not regulate quality, potency, or sterility of these products. Compounding pharmacies are typically permitted to prepare versions of FDA-approved drugs that are in shortage — but retatrutide isn't an approved drug, so the shortage pathway doesn't apply, and the legal status of these products is more uncertain than for compounded tirzepatide or semaglutide.

What the Trial Protocols Allowed for Dose Adjustments

Here's what the published TRIUMPH protocols actually permitted:

Holding a dose: Each step was held for at least 4 weeks before escalation. Trials did not test escalating sooner.
Permanent dose reductions: Allowed for participants experiencing gastrointestinal adverse events that were not improving, or for participants whose BMI dropped to ≤22 kg/m² or who experienced what investigators considered excessive weight loss.
Discontinuation: Permitted at any time at the participant's or investigator's discretion. Roughly 18% of TRIUMPH-4 participants on 12 mg discontinued due to adverse events; 12% on 9 mg.

Split Dosing

Some compounded retatrutide users describe splitting their weekly dose into two or three smaller injections through the week. The community rationale is that flatter peak drug levels may reduce nausea and the heart rate signal — same total weekly amount, lower Cmax. There is no published clinical data on split dosing of retatrutide.

Regulatory Context

On Lilly's Q4 2025 earnings call, the company guided to filing FDA applications for retatrutide in obesity, obstructive sleep apnea, and knee osteoarthritis at some point during 2026, pending the remaining TRIUMPH Phase 3 readouts (TRIUMPH-1, the pivotal obesity readout, was expected later in Q2 2026 per the Q1 2026 earnings call). With a standard FDA review window — and priority review plausible given the combined obesity / OA pain data — an approval decision in 2027 is on the table. Until then:

Retatrutide is not approved by the FDA for any use.
It does not have an approved label, approved dose, or approved indication.
Any retatrutide reaching consumers in the US comes from compounding pharmacies or research-peptide markets. Quality, sterility, and potency are not under FDA oversight.
The shortage-based compounding pathway that has applied to tirzepatide and semaglutide does not apply to retatrutide, since there is no approved reference drug.

How Does Foundayo Work?

Foundayo is a GLP-1 receptor agonist — the same basic mechanism as semaglutide. It binds to GLP-1 receptors in your brain and gut, suppressing appetite, slowing digestion, and helping regulate blood sugar. If you're familiar with how Wegovy or Ozempic works, you already understand most of what Foundayo does.

The key difference is how it's made. Semaglutide and tirzepatide are peptides — large, complex molecules that have to be injected because stomach acid would destroy them before they could be absorbed. Foundayo is a small molecule, which means it's more like a traditional drug in pill form. Your gut can absorb it intact.

A few practical advantages that come from this:

No food restrictions. Oral semaglutide (Rybelsus) requires a 30-minute fasting window before and after taking it, which is awkward for a lot of people. Foundayo has no such requirement — take it whenever.
No refrigeration. Small molecules are typically more stable at room temperature, which matters for travel and storage.
No injection anxiety. For some people, this is genuinely a big deal.

What it is not: a dual agonist. Tirzepatide hits both GLP-1 and GIP receptors, which is why it tends to produce stronger weight loss results. Foundayo is GLP-1 only, so it's more comparable to semaglutide than to tirzepatide in terms of mechanism.

FDA-Approved Dosing Schedule

The approved label uses different dose strengths than what was tested in the Phase 3 trials — the commercial formulation is reformulated at lower milligram strengths. The titration schedule below is what's on the actual Foundayo label:

A few things worth noting:

The approved dose strengths are significantly lower in milligrams than the trial doses (which went up to 36 mg). This isn't a weaker drug — it's a reformulated version with different bioavailability. Don't compare the numbers directly to the trial doses.
The 9 mg, 14.5 mg, and 17.2 mg doses are optional increases, not mandatory targets. Some people will reach a good maintenance dose before the maximum.
Not everyone needs to reach 17.2 mg. As with tirzepatide and semaglutide, some people see good results at lower doses and there's no clinical reason to push higher if you're doing well.
This is a daily pill, not a weekly injection — consistency matters. Building the daily habit is important.

How Does It Compare to Injectables?

This is the honest answer most people want: Foundayo is less effective for weight loss than the leading injectables, but it closes the gap meaningfully — and it's a pill.

The ~11–12% weight loss figure comes from the ATTAIN-1 trial at the highest dose: 12.4% among completers, 11.1% on an intent-to-treat basis (about 27 lbs and 25 lbs respectively).

What this means practically:

If you're currently on tirzepatide (Zepbound or compounded), Foundayo would likely be a step down in efficacy. It's not a direct swap.
If you're on semaglutide (Wegovy or compounded), the efficacy is closer — Foundayo loses some weight loss effectiveness but wins on convenience and, notably, lower nausea rates.
The nausea difference is real. GLP-1s are notorious for nausea, especially early in titration. Foundayo's GI profile skews more toward diarrhea and indigestion rather than nausea, which some people will find much easier to tolerate.

One surprisingly interesting data point: The ATTAIN-MAINTAIN trial specifically looked at people who switched from injectable GLP-1s to Foundayo. People who switched from Wegovy maintained about 95% of their prior weight loss. People who switched from Zepbound maintained about 80%. So if you're stabilized on an injectable and switch to the pill, you're likely to hold most of your progress — you're not starting from scratch.

What About Side Effects?

The approved label lists the most common side effects as: nausea, constipation, diarrhea, vomiting, indigestion, abdominal pain, headache, abdominal bloating, fatigue, belching, heartburn, gas, and hair loss.

A few things worth knowing about the profile:

Less nausea than semaglutide. The 13–16% nausea rate compares favorably to Wegovy's ~44%. This is one of the more appealing aspects of Foundayo for people who struggled with nausea on semaglutide.
More diarrhea than you might expect. The trade-off seems to be that diarrhea and dyspepsia are a bit more prominent with Foundayo. Not severe for most people, but worth knowing going in.
Hair loss is on the label. This is worth flagging because it's not commonly listed for the injectables. It's listed as a common side effect here.

The label also includes serious risks to be aware of: pancreatitis, severe GI problems, dehydration-related kidney issues, hypoglycemia (particularly relevant if you're also on diabetes medications), allergic reactions, vision changes, and gallbladder complications. These are serious but uncommon — the same category of risks that apply to GLP-1s generally.

Discontinuation rates are slightly higher at the maximum dose. At the highest label dose, somewhere in the 5–10% range of trial participants stopped due to side effects.
The standard advice applies: go slow, stay hydrated, eat smaller meals, and give your body time to adjust at each dose before moving up.

FDA Approval and Availability

The FDA approved Foundayo on April 1, 2026 — 50 days after Lilly's filing, making it the fastest New Molecular Entity approval since 2002. It was approved under the Commissioner's National Priority Voucher (CNPV) pilot program, which is designed to fast-track medicines addressing significant unmet medical need. No Advisory Committee meeting was required, which is generally read as the FDA finding the safety and efficacy data clean and straightforward.

Foundayo is approved for adults with obesity (BMI ≥ 30), or adults who are overweight (BMI ≥ 27) with at least one weight-related comorbid condition.

Availability:

LillyDirect started accepting prescriptions immediately following approval, with first shipments going out April 6, 2026. Broad retail pharmacy and telehealth availability is expected shortly after. Lilly reportedly stockpiled $1.5 billion of inventory ahead of the launch specifically to avoid the supply shortages that plagued semaglutide and tirzepatide in 2023–2024.

What This Means If You're on Compounded GLP-1s

This is the practical question for a lot of people on this site.

First: Foundayo is almost certainly not compoundable. It's a patented small molecule, not a peptide. Compounding pharmacies have made compounded semaglutide and tirzepatide available partly because the FDA's shortage list created a legal pathway. Foundayo isn't on any shortage list, it's not a peptide, and Lilly has clearly planned for adequate supply. There is no realistic compounding pathway here.

Second: it's not a replacement for compounded tirzepatide. If you're getting 15–20% weight loss on tirzepatide, switching to Foundayo would likely mean accepting meaningfully less weight loss (11–12%). Plus you lose the GIP agonist effect. If tirzepatide is working well for you, this isn't an upgrade or even a lateral move.

Where Foundayo could make sense:

You're currently on compounded semaglutide and losing access (due to shortage list changes, pharmacy closures, etc.). The efficacy profiles are closer, and Foundayo has the pill convenience advantage.
You haven't started a GLP-1 yet and hate injections. If a pill gets you to actually start treatment vs. avoiding injections indefinitely, that's a real benefit.
You've had significant nausea issues on semaglutide and the lower nausea rate of Foundayo might make it more tolerable.
You travel frequently or have storage constraints that make injectable medications annoying.

On cost vs. compounded: Compounded semaglutide and tirzepatide are typically $100–$200/month from telehealth providers, often less.
